Overview

The Clinical Dietitian is responsible for the evaluation, planning and clinical assessment of nutritional status of the residents and participants of Loretto long-term care facilities or sites. Coordinates all phases of nutrition care including nutrition assessment, care planning, monitoring, education and counseling of residents and participants in a variety of settings. Performs duties in compliance with licensing, accrediting, and regulatory agencies. Contributing member of the interdisciplinary team in establishing and implementing the interdisciplinary care plan.

Responsibilities

Job duties specific to this position:

  • Complies with all State and Federal safety, health and sanitary regulations as well as corporate policies and procedures.
  • Collaborates with providers, residents and participants to develop nutritional plans of care.
  • Completes initial nutritional evaluations of residents/participants, assesses and monitors nutritional risk.
  • Reviews P.O. intakes, lab results, residents/participants, charts/med history.
  • Addresses acute clinical situations, lab results, or social changes in a resident's or participant’s status, as they arise.
  • Reviews and revises nutritional care plans and schedules as needed.
  • Coordinates changes in nutritional care planning with staff, MDs, nurses, food service, etc and makes regular follow-ups.
  • Makes recommendations to providers regarding orders, monitoring and tolerance for enteral and parenteral nutrition. Completes follow up assessments monthly or when indicated.
  • Assists and guides Diet Technicians with lab evaluations, diet planning, and care planning for residents.
  • Assesses all residents/participants with identified pressure ulcers and completes follow up assessments monthly until healed.
  • Educates residents, participants, and family regarding prescribed diet and nutrition plan of care. Provides written materials to families, residents/participants, and/or caregivers, when appropriate. Assesses compliance and understanding of diet restrictions.
  • Assists in menu planning and recipe analysis. Communicates resident's and participant's comments regarding meal acceptance.
  • Participates in the Quality Assurance Performance Improvement (QAPI) programs, surveys, audits and correction plans.
  • Responsible for the training and orientation of newly employed staff and newly employed Dietetic Technicians and Clinical Dietitians. Leads training for staff members on nutrition related topics.
  • Keeps abreast of necessary developments in the field of gerontological nutrition and conveys this knowledge through nutrition education programs for participants and staff.
  • Acts as preceptor to dietetic students from area colleges/universities.
  • Assumes responsibilities of the Clinical Nutrition Supervisor in their absence (when applicable). Assumes responsibility for staff consultation in the absence of the Dietetic Technician.

Qualifications

  • Two years of clinical experience in a health care setting desired. Long-term care or outpatient experience preferred.
  • Completion of a dietetic internship.
  • B.A. or B.S. in dietetics from an AND (Academy of Nutrition and Dietetics) accredited academic institution, required.
  • Current registration or registry eligible.
  • Able to effectively assess residents or participants’ nutritional needs and establish plans of nutritional care.
  • Ability to work effectively in a collaborative environment.
  • Ability to communicate effectively with participants, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments. Excellent oral and written communications skills required.
  • Proficient in MS Office and able to learn new software rapidly.
