Overview

In collaboration with the Day Center Managers, assists in coordinating daily functions in the day centers.  Is responsible for monitoring of CHHA’s to assure assignments are completed and reports any issues to Day Center Manager.  Also supports the Day Center Managers by performing a variety of day center functions.

Responsibilities

PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS

Job duties specific to this position:      

  • Monitors completion of the aides’ daily assignments; reports aberrancies/concerns to the Day Center Managers.
  • Documents completed assignments in the EMR through monitoring and reviewing of daily assignments
  • In the absence of the Day Center Manager, directs and supervises Day Center Aides to assure completion of assignments and intercedes with interruptions in workflow throughout the day.
  • As necessary, reports acute issues of Day Center participants to clinic personnel.
  • Assists with daily attendance and follows up on “no call no show” issues. Assists with updating attendance schedule in EMR.
  • Greets and tours visitor; including reserving a place setting for lunch as indicated.
  • Assists in updating participant care plans based upon changes in MD orders or care plan recommendations from the IDT. Communicates changes in participant care plans to day center staff.
  • In collaboration with Education Services, assists in coordinating in-service training and orientation for Day Center staff.
  • Assist in coordinating transportation with Director of Transportation or designee to meet the needs of participants.
  • Is responsible for monitoring and assisting with inventory and ordering of supplies for the appropriate day center.
  • Maintains contact with contracted social day center as directed/needed by IDT.
  • Supports Recreation Therapy and Rehabilitation programming in the Day Centers.
  • Establishes and maintains cooperative relationships among staff; supports Director of Site Operations in the absence of the Day Center Manager, i.e., coordination of center and daily functions.
  • Demonstrates adherence to all compliance policies and procedures. Is responsible for promoting and fostering compliance in the workplace. 
  • Ensures the highest quality of care by performing responsibilities according to the highest professional standards.
  • Practices and teaches safety awareness. Identifies and reports or corrects any possible safety or environmental issues.
  • Demonstrates knowledge of PACE/D&TC Emergency Preparedness Plan and care verbalize their role during an event.

Qualifications

  • Minimum of one (1) year's experience in providing service to LTC or frail elderly clients in a health facility or community based setting.
  • Associates degree required.
  • Prior supervisory experience preferred.
  • Must have a reliable vehicle and valid driver's license.
  • Demonstrated ability to plan, organize, and complete duties independently.
  • Exceptional verbal and written communication skills.
  • Proficient in MS Office programs (Word, Excel) and able to learn new software rapidly.

An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
