The Nurse Screener RN case manages and coordinates services for all hospitalized, nursing home and rehabilitation residents. This position is responsible for post discharge screening for appropriate placement within the Loretto system and coordination of services for ongoing case management, using input from hospital records, patients and family members, physicians, nursing staff, therapists, payor organizations and any other appropriately licensed source of information. Effectively utilizes nursing assessment skills to complete necessary admission documentation for all admissions and readmissions.
The Nurse Screener RN communicates with hospital, Skilled Nursing Facility (SNF) and Adult Care Facility staff as needed for effective case managing. Performs assessments and home care coordination to minimize hospital lengths of stay, and promotes a smooth transition for the resident as he/she moves from the hospital to alternative levels of care.
The Nurse Screener RN implements and assesses appropriate resident care using best practice standards with Enriched Housing, Assisted Living Program, Assisted Living Residence and Licensed Home Care Agency regulations. Establishes and maintains cooperative working relationships with other program staff, outside agencies, providers and hospitals.
• 1-2 years’ experience in an adult care facility or home care.
• 1-2 years’ full time nursing experience, preferably long term/acute elder care.
• Current NYS license and registration in good standing as a professional nurse
• Must possess current and unrestricted driver's license and a reliable vehicle with insurance.
• Graduate of an accredited school of nursing. BSN or BS in related field preferred.
• Knowledge of relevant DOH regulations.
• Proven assessment and care planning skills.
• Current knowledge of best practice nursing standards and care.
• Ability to work independently and as part of an interdisciplinary team.
• Outstanding interpersonal and communication skills, including the ability to communicate
effectively with clients, residents, and families from diverse backgrounds who may have
physical, sensory and mental impairments.
• Effective problem solving and conflict resolution skills.
• Proficient in MS Office and able to learn new software rapidly.
An equivalent combination of education and experience which provides proficiency in the areas
of responsibility listed above may be substituted for the above education and experience
requirements.
