Overview

The primary purpose of the Medicaid Eligibility Coordinator is to assist potential PACE CNY participants and families in the application process for Medicaid. This is a supportive role to the Intake Department to ensure that we have all appropriate documentation and forms for the Medicaid application.

Responsibilities

Job duties specific to this position:

  • Search potential participants in e-PACE, run an eMedNY to determine type of Medicaid examples: chronic, community, mainstream, etc if they have a Medicaid CIN.
  • Coordinate the Medicaid application process for participants by working directly with the RN Intake Specialists or Intake Screener, maintaining ongoing communication, and providing status updates to the Director of Intake & Marketing.
  • Responsible for maintaining a log of Medicaid Eligibility.
  • Work with potential PACE participants over the phone and in person to go over required Medicaid documents.
  • Make home visits to acquire required documentation.
  • Travel to locations within Onondaga County to obtain documents, if necessary.
  • Meet or exceed deadlines of the department by uploading documentation to the Loretto/PACE Admissions Finance Manager. Sometimes delivery of hard copies to the Loretto Finance Braun Building.
  • Demonstrates adherence to all compliance policies and procedures.
  • Other duties as assigned.

Qualifications

  • Associates degree in Public Health, Human Resources – Benefits, Business or other related field.
  • High School Diploma or equivalent required.
  • Minimum of 1-2 year’s experience working with special needs or elderly population preferred.
  • Must have a reliable vehicle and valid driver’s license.
  • Responsible “self-starter” who keeps commitments to deadlines.
  • Must be action-oriented, be participant (customer) focused, have flexibility to adapt to ongoing change and good organizational and time management skills.
  • Knowledge of the Medicaid system and benefits; have experience in Medicaid operations and understand financial assets such as bank accounts, IRAs, life insurance.
  • Clearance of a background check.

 

An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
