Overview

The Sr. Quality Improvement Analyst directs, analyzes, researches and understands critical data points to improve clinical competencies and quality assurance programs of Loretto’s PACE program. Provides guidance, support, communication and coordination of clinical programs, goals, standards and applications. Reviews all quality data to ensure accuracy and assist with the development of quality improvement plans. The Sr. Quality Improvement Analyst assists the Director of Quality Management in evaluating, trending, and summarizing all aspects of services to assure quality and to identify opportunities for improvement, as well as ensuring compliance with applicable Federal and State regulations and Corporate Policies.

Responsibilities

Job duties specific to this position:

  • Conducts chart audits for PACE and/or D&TC and/or Licensed Agency.
  • Assists in the development of indicators and as directed reports during quarterly meetings.
  • Prepares audit tools for Medical Director’s review including, but not limited to Complaints, Polypharmacy, Adverse Incidents.
  • Assists in maintenance of Grievance and Appeals Logs and files.
  • Assists in maintaining Adverse Incident Report Log and files.
  • Assists the Director of Quality Management in ongoing opportunities for Quality Improvement initiatives that identify program performance issues.
  • Identifies trends and informs QA Director.
  • Assists in compiling data for transmission to appropriate regulatory agencies and submits data as directed.
  • As directed, attends morning team meeting
  • Responsible for collecting and entering all data from appropriate supervisors for the Quality Report Card.
  • Responsible for conducting and assists in the evaluation of Customer Satisfaction of Participant and Family Surveys annually and as recommended.
  • Responsible for the completion of quarterly personnel record audits and follow up activities with human resources.
  • Assists in the collection of information for Utilization Review.
  • Assists in monitoring implementation of the statement of deficiency and plan of correction.
  • Assists in the development of monitoring tools and design data collection procedures, methods, and systems to track and validate findings.
  • Communicates qualtiy standards/problems/issues to staff and management as required by regulations.
  • Maintains communication of events with appropriate contracted agencies and informs organization management of urgent issues on a continuum.
  • Assists with maintaining agendas and taking minutes for quality department meetings.
  • Assists with maintaining QI (Quality Improvement) departmental standard operating procedure manual).
  • Demonstrates adherence to all compliance policies and procedures. Is responsible for promoting and fostering compliance in the workplace.
  • Analyzes/trends the monthly audits.
  • Ensures the highest quality of care by performing responsibilities according to the highest professional standards.
  • Demonstrates knowledge of PACE/D&TC Emergency Preparedness Plan and can verbalize their role during an event.

 

Qualifications

  • Bachelor's Degree or higher required in a health and/or human service field and at least five-seven years of experience in homecare, long term care or managed care
  • RN licensed preferred.
  • A valid and clean New York State Driver’s License and reliable vehicle with appropriate insurance required.
  • A positive customer focused attitude.
  • Experience with and knowledge of quality assurance and improvement activities and process improvement techniques, Lean Six Sigma Certification preferred.
  • Proficient in MS Office, particularly Word, Excel, and data collection/management tools.
  • Able to learn new software rapidly particularly an EMR.
  • Effective communication skills (verbal, written, listening).
  • Willingness to be flexible and perform extra duties as needed.
  • Ability to work effectively within a team and also independently.
  • Ability to multitask and meet deadlines.
  • Knowledge of CMS, NYS DOH regulations which apply to MLTC, PACE, D&TC’s, Licensed or Certified Home Care Agencies highly desired.
  • Knowledge of evidence based practices and quality principles with at least 5- 7 years applicable experience
