Join Loretto Health and Rehabilitation as a Full Time Clinical Coordinator, RN! 

 

Under the guidance of the Unit Nurse Manager, The Clinical Coordinator RN is responsible for coordinating the clinical practice and care to residents in a Long Term Care and Short Term Rehab setting. This role is a liaison between residents, families, medical providers and Loretto employees. Conducts patient assessments, implements and evaluates the effectiveness of nursing care delivered to the residents.

Shift: Full Time Days 7:30am – 4:00pm or 8:00am – 4:30pm 

Monday - Friday (No Weekends)

Pay: $84,323.00 - $94,702.00 Yearly Salary

Responsibilities

Responsibilities include:

  • Oversee the delivery of Quality Patient Care
  • Compliance with NYSDOH and CMS regulations and facility policies and protocols
  • Orients, trains and mentors new employees
  • Provides health training to patients and their families
  • Assists in gathering and analyzing data for QAPI
  • Acts as primary liaison with medical providers

Qualifications

  • 1+ year experience in nursing preferred
  • Current Registered Nurse NYS license
  • Established patient assessment skills
  • Knowledge of best-practice standards
  • Skilled to respond to emergent situations
  • Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes
  • Exceptional interpersonal and customer service skills
  • Proficiency in speaking, listening, reading and writing the English Language is required
