Overview

We are now hiring a Primary Care Physician to follow an assigned panel of patients in collaboration with their interdisciplinary team!

 

PACE CNY, a unique Program of All-inclusive Care for the Elderly, is a Medicare Managed Care program providing care for individuals that meet criteria of 55 years and older, living in Onondaga County who are in need of long term care.  The philosophy of the program is to maintain participants' independence in the community for as long as medically and socially feasible and continue community residence with family support and minimal disruption of the  participants life.  

Responsibilities

Additional PACE CNY Program Details:

  • Program serves as the primary care service and the insurance carrier for its patients (participants)
  • Interdisciplinary team includes Nursing, Social Services, Occupational Therapy, Physical Therapy, Recreational Therapy, Nutritional Services, Transportation, Day Center Staff and Home Care.
  • Basic criteria for an enrollee includes being at least 55 years old, a resident of Onondaga County, and in need of the plan's long-term care services for a period of time greater than 120 days.

  

Provider Position Details:

  • Assumes responsibility for the primary medical care of PACE participants and provides direct medical care including evaluation, treatment of episodic and chronic illnesses, and end of life care, etc. Also provides primary medical care to participants who require nursing home placement; performs home visits and nursing home visits as appropriate.
  • As a member of the interdisciplinary team, performs pre-enrollment assessment of potential enrollees; performs extensive initial medical evaluation on new participants; develop a treatment plan in cooperation with participants, family, and team members; perform a semiannual assessment, and develop an individualized plan of care for the next six months; participates in family meetings, nursing home care planning meetings, and clinical case conferences as appropriate; participates daily in morning meetings.
  • Actively participates or may chair committee meetings such as Pharmacy and Therapeutics, Root Cause Analysis/Level II investigations, staff, and provider meetings, etc.
  • Active in QAPI program; participates in QA/Utilization review activities; QAPI task forces as appropriate.
  • Hours are typically 8 AM to 5 PM, Monday through Friday at our day center site
  • Typical day consists of patient visits and team meetings at the day center
  • Typical caseload is 80-100 participants, seeing 5 to 15 patients in the clinic per day.
  • On-Call (which occurs with team support) is managed by phone calls, does not result in travel, and is distributed among the providers.
  • EMR is TruChart and full training provided
  • We are not fee-for-service. We do not have “quotas.”  Our “billing” is based on our patients’ risk scores and reflects a managed care program.
  • Our providers are very supported, and have access to multiple content expert colleagues who can assist in patient care. PACE is a collegial operation and is very committed to providing high quality care. 

Qualifications

  • Graduate of accredited medical school and post graduate residency program with current license to practice medicine in New York State OR Current certification as a Nurse Practitioner in NYS and an advanced nursing degree
  • Board certification in Internal Medicine granted by the American Board of Internal Medicine or in Family Practice granted by the American Board of Family Practice.
  • Current DEA privileges.
  • Demonstrated experience in caring for geriatric patients. Must have the knowledge and skills to assess, plan care for and provide daily services to frail, elderly program participants.
  • Additional desired qualifications include:
    • Certificate of added qualifications in Geriatric Medicine granted by the American Boards of Internal Medicine or Family Practice.
    • Fellowship training in Geriatrics.
    • Considerable experience in long term care setting, (e.g. home care programs, nursing home care, chronic care hospital) and in acute care setting, (e.g. acute hospital, emergency room).
  • Strong interpersonal skills and effective verbal and written communication required. Willingness to work in an interdisciplinary setting necessary. Must be able to read, write and speak the English language.
  • Proficient in MS Office and EMR with the ability to learn new software rapidly.

An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
