The Clinical Coordinator, RN is responsible for coordinating the clinical practice of staff and following up on areas of concern to ensure holistic care is provided to each resident.

The Clinical Coordinator, RN (Registered Nurse), serves as a liaison between the resident, family, physician, and Loretto staff, provides daily assessment of each resident, and communicates changing needs to appropriate personnel.

The Clinical Coordinator also serves as a liaison between clinic staff and reports to a Director of Site Operations, Program Director, or Nurse Manager.

 

Shift: Full Time Days 8:00am – 4:30pm

Monday - Friday --- Occasional Weekend Required

 

Pay Range: $84,323.00 - $94,702.00 annually

 

Responsibilities

Job duties specific to this position:

  • Directly oversees nursing care provided to patients or participants in a department, function or site. Assigns and directs staff’s daily work and all day to day activities. Follows up to ensure duties are carried out.
  • Functions as a staff RN when necessary.
  • Facilitates communication of vital information to all staff as necessary.
  • May work with a secretary to ensure patients are scheduled/rescheduled for appointments and tests as necessary.
  • On a daily basis, reviews all required documentation for completion and accuracy.
  • Monitors and ensures the proper use of equipment in assigned area. Provides instruction or obtains maintenance and repairs as needed.
  • Monitors medication and other patient related errors. Takes action to minimize occurrence and follows up appropriately per policy and/or regulations.
  • Monitors follow through on provider’s verbal and written orders to ensure completion including lab work, consults, appointment scheduling, rescheduling and follow up etc.
  • In working with the team, ensures the unit is in compliance with NY State Department of Health regulations and in compliance with corporate and regulatory policies and procedures; addresses issues appropriately.
  • Orients and trains new clinical staff, ensuring all clinical and regulatory requirements are met and the orientation and competency paperwork has been completed.
  • Provides health training to participants, family and caregivers as required; assesses participant abilities and suggests interventions to maximize independence and quality of life.
  • Ensures environment is clean and safe for patients by conducting rounds. Initiates corrective actions as required. Assists in the monitoring of compliance with QA checks (refrigerator, code cart, etc.) and works with the nurse manager to ensure policy adherence.
  • Acts as primary liaison for team and contracted providers; provides direct input into patients’ care plans as appropriate, communicates changes for treatments/medication to interdisciplinary team and contracted providers and care givers as appropriate.
  • Completes own tasks promptly and ensures staff completes tasks in a timely manner.
  • Identifies systems/processes in need of improvement and makes recommendations to Nurse Manager.

Qualifications

  • 1 year of nursing experience in long term/acute care setting preferred.
  • Graduate of an accredited school of nursing. BSN or BS in related field preferred.
  • Current New York State RN license and registration in good standing required.
  • Proven patient assessment and clinical skills.
  • Current knowledge of best practice nursing standards and the care planning process.
  • Ability to manage and guide staff while carrying out the duties of an RN.
  • Ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.
  • Proficient in computer programs and able to learn new software rapidly.
  • Proven organizational and time management skills with the ability to multitask. 
  • Ability and knowledge to respond in emergency situations.
  • Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes.
