The Home Health Aides Manager at Sedgwick Heights (1100 James St, Syracuse, NY 13203) is responsible for ensuring safe, effective, and efficient care is delivered by the Home Health Aides.

The manager provides leadership and support to staff providing clinical services. Observes, plans, implements, and evaluates the effectiveness of resident care delivery.

Assures adherence to corporate policies, procedures, and all regulatory guidelines for all CHHA (Certified Home Health Aide) staff.

Provides guidance and support to the CHHA staff, including orientation, training, competencies, and supervision.

Manages follow-up for after-hours calls from residents, families, staff, and others as needed. 

 

Weekdays: 3:30pm – 12:00am or 4:00pm – 12:30am

Weekends: 2:00pm – 10:30pm

Requires Working Every Other Weekend, Saturday & Sunday 

 

Pay Range: $29.05 - $30.28 per hour

 

NY State Certified Home Health Aide Certification Required

LPN License Preferred

Responsibilities

  • Oversees and is responsible for the home health aide staff and scheduling. Ensures resident care needs are met with high-quality care.
  • Manages staff: interviews, hires, orients, evaluates, educates, and counsels home health aide staff.
  • Ensures accurate and timely completion of CHHA documentation in relation to DOH guidelines.
  • Completes a written performance evaluation of staff supervised: at the end of the three-month probationary period; and once per year on or within four weeks of the staff member’s anniversary date. The evaluation includes an assessment and an observed demonstration of the delivery of care.
  • Responsible for decision-making and problem-solving in regard to home health aide concerns and issues.
  • Responsible for decision-making and problem-solving in regard to resident and/or family concerns and issues.
  • Interprets and applies policies, procedures, regulations, and the Union Contract in all instances
  • Works with the Clinical Care Manager, Human Resources, and Union representative in situations requiring discipline, suspensions, and terminations
  • Exercises sound judgment based upon acquired knowledge, resident need, staff need, and program service scope. Is self-directed in daily routines in support of program objectives
  • Effectively communicates and collaborates with the Care Management team
  • Guides and provides leadership to the scheduling department during challenges as needed
  • Must ensure confidentiality of information at all times.
  • Ensures employee follows established safety guidelines.
  • Attends and runs staff and in-service meetings as required.
  • Includes on-call and other duties as assigned.

  • Fosters process improvements for greater operating efficiency and resident/customer service. Ensures optimal workflow and adequate resources.
  • Actively leads or participates in meetings, teams, and task forces as requested.
  • Ensures adequate skills, training, and development among staff.
  • Manages and supervises assigned personnel, including performance management, scheduling, and orientation. With the Director and Human Resources guidance, makes or approves recommendations on employee hires, transfers, promotions, salary changes, disciplinary actions, terminations, and similar actions. In conjunction with HR, resolves grievances and other personnel problems in a fair, timely, and consistent manner.
  • May have input to or recommendations for operating budgets.

Qualifications

  • NYS Certified Home Health Aide Certification Required
  • LPN preferred, but may be waived based on experience.
  • Two years of patient care experience required.
  • 1-2 years of previous experience in supervision, mentoring, or training preferred.
  • Excellent communication skills, including good telephone etiquette.
  • Strong leadership, time management, and interpersonal skills.
  • Strong organizational skills with attention to detail.
  • Demonstrates motivation and initiative in creative problem-solving.
  • Proficient in MS Office, including Word and Excel, and able to learn new software rapidly.
