The Clinical Care Housing Manager is responsible for ensuring that safe, effective, and efficient clinical care is provided by the clinical care team, which consists of LPNs, Case Managers, Enriched Living Supervisors, Home Health Aides, and outside providers.

The Clinical Care Housing Manager provides leadership and support to staff delivering clinical services, observes, plans, implements, and evaluates the effectiveness of resident care, and communicates information appropriately to the interdisciplinary team.

 

Schedule:

Monday - Friday 8:00am - 4:30pm

No Weekends!

 

Salary: $74,000.00 - $79,000.00 annually

 

Responsibilities

Resident/Client Care:

  • Supervises all clinical activities of the interdisciplinary team, which may include Licensed Practical Nurses, Case Managers and direct care aides.
  • Oversees staff to ensure residents’ care needs are met.
  • Acts as an integral part of daily rounding.
  • Monitors clinical staff for adherence to all corporate policy, procedure and all pertinent regulatory guidelines.
  • Manages/schedules staffing levels to provide effective care to residents.
  • Collaborates with specialty service coordinators/outside providers regarding paraprofessional or specialty service issues. (Physicians, hospitals, etc.)
  • Ensures adherence to ALP, Enriched Housing, Assisted Living Residence and Licensed Agency regulations.
  • Facilitates and provides clinical coordination with on-call staff.
  • Assists staff in developing a care plan with direct input.
  • Ensures the implementation of physician change orders, scheduled lab work, consults and appointments.
  • Ensures completion and accuracy of all nursing documentation.
  • May provide direct care when needed.

Employee Development:

  • Provides for appropriate and effective orientation of all staff.
  • Serves in the absence of the Administrator in their capacity as directed.
  • Acts as an integral part of the interdisciplinary team.
  • Keeps self and staff current in best practices nursing care. Participates and encourages participation in professional associations, educational conferences, and in-services as appropriate or required.

Quality Assurance:

  • Maintains an effective system for resident complaint/issue resolution.
  • Monitors staff effectiveness and initiates corrective actions in collaboration with the Administrator and the Director of Clinical Services as needed for clinical issues.
  • Actively participates in the development and monitoring of QA/QI metrics in collaboration with the Quality Assurance lead.

Compliance:

  • Monitors and ensures staff adherence to HIPAA confidentiality requirements.
  • Ensures that all pertinent DOH and Agency regulations are adhered to by the clinical/direct care staff.
  • Takes calls and related responsibilities.

Job Duties as a Manager:

  • As a Manager, this position is responsible for the ongoing operations and routine decision-making within the function, department or site. Emphasis is on executing strategy and tactics. May provide input on strategic matters.
  • Interprets and applies organization policy, ensuring all activities are in compliance.
  • Delegates, organizes and coordinates the daily operations of departments within the function, department or site.
  • Promotes and encourages teamwork among employees and supervisors, and between departments. Communicates ideas and goals clearly and is an effective listener. Ensures that information is effectively communicated and shared throughout the department and organization. Provides and encourages effective coaching. Promotes a positive environment of constructive feedback.

  • Fosters process improvements for greater operating efficiency and resident/customer service. Ensures optimal workflow and adequate resources.
  • Actively leads or participates in meetings, teams and task forces as requested.
  • Ensures adequate skills, training and development among staff.
  • Manages and supervises assigned personnel, including performance management, scheduling, and orientation. With the Director and Human Resources guidance, makes or approves recommendations on employee hires, transfers, promotions, salary changes, disciplinary actions, terminations and similar actions. In conjunction with HR, resolves grievances and other personnel problems in a fair, timely and consistent manner.
  • May have input to or recommendations for operating budgets.

Qualifications

  • Licensed Practical Nurse required, Registered Nurse Preferred
  • 5+ years of healthcare experience.
  • 2+ years of operations experience in a supervisory role.
  • Graduate of an accredited school of nursing. BSN or BS in related field preferred.
  • Licensed and registered in good standing in New York State.
  • Current NYS driver's license and dependable vehicle.
  • Proven knowledge of adult care facilities and applicable DOH regulations.
  • Current knowledge of best practice nursing standards and care.
  • Proven evaluation and care planning skills.
  • Ability to lead, guide and motivate staff.
  • Outstanding interpersonal skills including the ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.
  • Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes.
  • Effective conflict resolution skills.
  • Proficient in MS Office programs and able to learn new software rapidly.
