The Enriched Living Supervisor LPN assesses the individual needs of the residents to ensure all personal care is maintained, that medications are taken as prescribed, and the physical, mental, social, recreational and spiritual needs of the residents are being provided for by the programs offered in the facility. With other colleagues, plans and provides for the overall comfort, safety, and well-being of the resident. Monitors and supervises staff providing personal care assistance to residents. Monitors adherence to all policy and procedures to assure compliance with DOH and all other pertinent regulations.   This is a Per Diem, Float LPN Supervisor position, floating between the different housing facilities in the Syracuse area, helping out with whatever the facility needs are. LPN can being doing anything from med pass to helping with resident care. The majority of the hours needed for picking up shifts can be either evenings/nights, some day shifts. The requirements to stay active would be working a minimum of six shifts a month, including two weekend shifts (shifts do not need to be on the same weekend).   Pay Rate = $31/hour

Under the supervision of the PACE Clinic Manager, the Clinic LPN is responsible for management of the medication program for the D&TC and PACE participants, including reordering of participants medications from pharmacy; conducting a variety of quality assurance functions per guidelines to monitor quality of services provided; completing variance reports as necessary; administering medications as ordered during day center hours of operation; and working closely with pharmacy personnel to ensure medication deliveries are accurate and timely.
