Under the guidance and direction of the Director of Patient Services and Home Care Manager, the After Hours RN will provide intermittent assessments, direct physical nursing care, and other home visits needed per the PACE CNY Clinic and the Answering Service/Triage RN Service. The After Hours RN is responsible for providing the highest quality of care in their home care and ensuring safety at all times according to accepted nursing practice standards. The RN acts as a primary support to the Home Care Manager. The practice of the profession of nursing as a Registered Professional Nurse is defined as diagnosing and treating human responses to actual or potential health problems though such services as case finding, health teaching, health counseling and provision of care supportive to or restorative of life and well-being, and executing medical regimens prescribed by a licensed or otherwise legally authorized Physician or Dentist. A nursing regimen shall be consistent with and shall not vary any existing medical regimen. (Education Law, Article 139)

The RN Home Care Manager at PACECNY (109 Lawrence Rd E. Suite 20 Syracuse, New York 13212) is responsible for ensuring the provision of high quality home care services. If you are not already certified, paid training is available!    Some Home Health Aide positions are in the community, while others are  located at 1 building, therefore, your own vehicle is not always required. Some locations are located on the bus route.  Locations that are not easily accessible by the bus route, include North Syracuse, Jamesville and Baldwinsville. CHHA shifts and sites will be mutually agreed upon prior to the start of class. Full time and part time positions are available.   Our training classes run monthly and is a paid training program with classroom and clinical hours from 7:00am - 4:30pm. Under the supervision of the PACE Clinic Manager,  the Clinic LPN is responsible for management of the medication program for the D&TC and PACE participants, including reordering of participants medications from pharmacy; conducting a variety of quality assurance functions per guidelines to monitor quality of services provided; completing variance reports as necessary; administering medications as ordered during day center hours of operation; and working closely with pharmacy personnel to ensure medication deliveries are accurate and timely.
